We have provided the teachers in Cebu Province this report before but it did not work out.

during submission of report, some principal and supervisors do not know how to read excel file or documents. Some even said it is unreliable since it is not hand written so they order the poor teacher to re write it with the regular form.

I realize that we should train and educate the leader first for them to realize the importance of technology. Trapo teachers are always hindrance to ICT development

I will be proposing in our school to make all form 137 encoded, because aside from the neatness, I really think that it is more reliable than handwritten forms. I couldn't agree more with the fact that we have to train and teach leaders the efficiency of this kind of system.

I am a Computer Teacher here in Lucban, Quezon and I must say that things like Technology should be applied in both fields. Teaching and applying (paper works). In these ways, less time, less effort and less redundancy. It's a big help for me...

I suggest a Consolidated Grade Reporting System, using a spreadsheet and/or office suite-based database frontend. I know, IT profs frown at these because it does not require much coding, but what is java, php or .net anyway? they can be classified as API's in a way that they're by-products of C.
Following the Agile SDLC flow, you got:
GRADES FROM SUBJECT TEACHER (for subjects/quarter) ==> SECTION COUNSELOR'S SUMMARY OF GRADES (one per student)
==> INDIVIDUAL STUDENT'S FORM 137 and DEPED FORM5.

But if your prof insists, use PHP/HTML5 to make it accessible by web or mobile devices.

Please perform regression testing with all possible use/test cases prior to deployment. Good luck!
